Criteria Requirements for Full Member

1 Criterion: Nature of the operations of the agency:
Measure: The agency is responsible for reviews at institutional or programme level of post-secondary
education institutions or post-secondary quality assurance agencies
Description: The legal foundation of IAAHEH are the following laws and regulations:

1. Law No. 12 year 2012 regarding Higher Education, article 55, paragraph 5 which mentions that
accreditation of study program as a form of public accountability is carried out by independent
accreditation agencies;

2. Minister of Research, Technology and Higher Education Decree No.32 year 2016 on
Accreditation for Study Programs And Institutions;

3. Minister of Education Decision No.291/P/2014 on Recognition of LAM‐PTKes/IAAHEH;
2 Criterion: Mission statement and objectives:
Measure: The agency has formulated a mission statement and objectives which are consistent with the nature of the agency
Description: LAM-PTKes’/IAAHEH Vision:

“To ensure global standards of quality for higher education in health”

LAM-PTKes’/IAAHEH Mission:

“To implement national accreditation of higher education in health that is sustainable and trustworthy”

LAM-PTKes’ Objectives:

1. Development of accreditation policies, standards, instruments, and procedures for higher education in health to be implemented by IAAHEH;

2. Quality improvement of study programs accredited by IAAHEH;

3. Integration of accreditation of academic, vocational, and profession levels of education to improve competencies of health workers comperehensively;

4. Competency of students graduating from study programs accredited by LAM-PTKes that meet standards and needs of society;

5. Financial independence of LAM-PTKes to operate.
3 Criterion: Agency staff, Numbers, Profile, Roles:
Measure: The profile of the staff is consistent with the Mission Statement
Description: IAAHEH Board of Trustee that include 14 representative from seven professional organization and seven from Professional Education Association. Each divisions have to manage accreditation of the study programs that relevant to the discipline. Beside that there is a Council of Accreditation, there are 15 persons as representative from a professional organization and educational profession that elected to be a member of council. And some supporting staffs to work in that positions as shown in the organization structure attached.
4 Criterion: Profile of reviewers:
Measure: The profile of the reviewers is consistent with the Mission Statement
Description: The IAAHEH’s evaluator are 638 persons that act as facilitator 106 health professionals, assessor 383 health professionals and 149 validators. They are from more than seven health professionals namely : Medicine (Generalist and Specialist), Dentistry (Generalist and Specialist), Nursing (Vocational, Profession and Specialists), Midwifery (Vocational), Pharmacy (Pharmacologist), Nutrition (Nutritionist), and Public Health. The evaluator team have chosen according to the criteria’s and the psychological test result. To maintain the objectiveness, it is very important consideration taking account that the team assigned to be facilitator or evaluator should be from different area or provinces.
5 Criterion: Independence:
Measure: The judgements and recommendations of the agency’s reports cannot be changed by third parties
Description: Decision Making on the Plenary Meeting of the Council Accreditation
(1) All decisions made by Council Accreditation on Plenary Meeting based on discussions and agreement;
(2) All that already decided in the meeting have to be written in the meeting minutes that have been signed by the leader of Council and the secretary also, all the attendance council members.

Decisions made by Agreement
(1) Decision making based on agreement have made after all the attendance members have chance to express their ideas and suggestions;
(2) The leader of council prepare the draft of decision that included all the ideas in the meeting;
(3) Decision making based on agreement is legal if the meeting attendance is quorum.

Result of Decision Making in the Meeting
(1) All that already decided in the meeting have to be written in the meeting minutes that have been signed by the leader of council and the secretary also, all the attendance council members;
(2) The council accreditation secretary have to write down the minutes meeting that consist of decision making about the status, rank, and score of accreditation of health study programs;
(3) The council accreditation secretary have to write down the documentations of the meeting that include conversation in the meeting;
(4) The council accreditation secretary have to write down the draft of documentation and minutes meeting to be deliver to the members and another important section in the meeting;
(5) Every members and all relevant part should be given chance to correct before the meeting closed by leader of council.
6 Criterion: Resources:
Measure: The agency has sufficient resources to run its operations in accordance with its mission statement and objectives
Description: The IAAHEH have sufficient human resources as well as facilities and infrastructure, and finance. And the operational of accreditation process are controlled by executive board and the board of trustee. There are some regulations that should be followed by all staffs. The operational of IAAHEH is also supported by government and National Accreditation Agency for Higher Education (BAN-PT).
7 Criterion: External quality assurance criteria and processes:
Measure: The description of the processes and criteria applied should be transparent and publicly available and normally include:
self evaluation, site visit, public report and follow-up measure
Description: The Indonesian Accreditation Agency for Higher Education in Health (IAAHEH) which in Bahasa Indonesia is called LAM-PTKes, is an independent body that started to operate in March 2015. The aim of accreditation is to comprehensively evaluate and assess the commitment of study programs toward quality and its capacity to carry out the triple duties of universities (education, research and service), which reflects the liability of the academic and professional/clinical stages of the study program. Evaluation and assessment done by assessor team, consists of assessors who are experts and/or experts in the implementation of health study programs. Decisions regarding quality are based on evaluation and assessment of evidences related to education standards already established and based on peer expert judgment. Evidence needed includes study program’s written reports which are verified and validated through the assessor team‘s visit to the study program (self evaluation, written report of study programs, written report of institutions).
Based on this legal foundation, the objectives and benefits of accreditation of Health Study Programs are as follow:
1. To provide assurance that the accredited study program has complied to quality standards already established to protect the community from study programs that have not complied to these standards
2. To encourage study programs to continually improve and maintain high quality
3. The result of accreditation may be used as consideration for student credit transfers, financial aids, and recognition from other institutions.
The score of evaluation is 100% that consist of:
a. self evaluation of study programs 10%
b. Written report of study program 75%
c. Written report of institution 15%.
The procedure of study program evaluation started from the initial requirement documents submitted to IAAHEH, followed by desk evaluation by assessor to check three documents above. The site visit have to be done after desk evaluation by at least two assessors.
The score of accreditation have decided by senate accreditation based on the score evaluation that submitted by assessor after site visit. The score will announce to the study program and institution and publish in the IAAHEH website (http://lamptkes.org/).
8 Criterion: Quality assurance:
Measure: The description of the processes and criteria applied should be transparent and publicly available and normally include:
self evaluation, site visit, public report and follow-up measure
Description: The quality assurance of IAAHEH was done by external examiner in the first year (2015), stakeholders (health higher study programs, National Accreditation Agency for Higher Education (BAN-PT). The data were collected to measure the customer satisfaction from 341 study programs of high health education, private and government institutions.
The data also are strengthen by focus group discussion with selected study programs. Evaluation of IAAHEH performance and documents have been done by National Accreditation Agency for Higher Education (BAN-PT).

Criteria Requirements for Institutional Member

Institutional Member Criteria:
Measure: Institutional Members are institutions of higher education in the region that are in good standing with the relevant quality assurance agency if one exists.

Criteria Requirements for Associate Member

Associate Member Criteria:
Measure:Associate Members are organisations with a major interest and active involvement in evaluation, accreditation and quality assurance in higher education, but without the responsibility for assuring the quality of institutions, education programs, or external quality assurance agencies
